What is a Form 471?

A Form 471 is the funding application an E-Rate applicant files after selecting a vendor from their Form 470 competitive bidding process. It contains Funding Request Numbers (FRNs) — one per service/vendor combination.

How to search

Enter a Form 470 application number (9 digits, e.g. 470123456) and click Search. The tool queries USAC's FRN status dataset and returns all matching Form 471 FRNs.

Natural language: You can type "Find 471s for app 470-2026-1234567" and the AI will extract the numeric application number.

Status meanings

  • ·Funded — USAC has committed the funding
  • ·Pending — application is under review
  • ·Denied — USAC denied the FRN (check denial reason)
  • ·Cancelled — applicant or USAC cancelled the request
